Auto Accidents and Slip & Fall

Intake form - new automobile accident questionnaire - in Adobe pdf format - use this form to describe what happened to you, identify the insurance companies, identify the other driver and witnesses and describe in your own words what happened in the accident. Once you have completed the form, bring it to my office or fax it to Jonathan Ginsberg at 770-234-5434.

Department of Motor Vehicles driving record - if you have ever wondered what is on your driving record from Georgia or any other state:  Click here to order your DMV printout

Georgia Dept. of Motor Vehicles Request for Motor Vehicle Record - this is a form provided by the State of Georgia where you can request your own driving record of someone else’s driving record. It is slightly less expensive than the service described above, but it requires you to enclose your own self addressed envelope.

Georgia Dept. of Motor Vehicle Personal Accident Report - this is a form similar to that used by the police for accident reportrs.

Social Security new client questionnaire - Social Security requires you to provide certain information about your past work, your medical treatment and your disabling condition. Our intake form, which is in Adobe pdf format, will help you organize this information. Bring your completed form to us, or, if it is more convenient, fax or mail it to us. Our fax number is 770-393-0240.

Appointment of Representative form - in order for our law firm to represent you, you will need to complete an “Appointment of Representative” form. You can download this form in Adobe pdf format.

Attorney-client fee contract - we handle most Social Security cases under contingency fee. This means that you pay no attorney’s fees unless we win a lump sum payment for you. You can download and review our fee contact here. Please note, however, that no attorney-client relationship is created until both you and one of our lawyers agree to the representation and both sign the fee contract and the Appointment of Reprensentative form. When we accept a case, we will mail you a letter confirming our representation and copies of the fee agreement and Appointment of Representative forms signed by both you and one of our lawyers. Since time deadlines are short for filing appeals and other documents with the Social Security Administration, you should not assume that we will take any action on your behalf unless and until you receive written confirmation from us that we are representing you.

Statement of Earnings and Benefits - use this form to request a copy of your earnings record from Social Security. Your record will also contain an estimate of how much you would likely receive in monthly payments if you become disabled or retire.
