Your new client intake form is the most important tool we use to assist you. Please complete this form as best you can and fax it to Bernie, Meg or me at 770-393-0240. One of us will review this form and then contact you to advise you how we see your situation and to discuss your options. I do not charge for reviewing your financial situation.

If we decide to proceed, we will have a head start on completing your paperwork - often we can have a draft petition in your hands (by email) prior to your office appointment.

If you have any problems downloading any of this paperwork, please call my assistant, Yolanda Gaston at 770-393-4985 and she will rush a hard copy to you by mail or fax.
